Clinical work includes the generality of plastic surgery including aesthetic surgery.


Mr Williams undertakes dedicated hand clinics including investigation and treatment of problems related to nerve injury and these are supported by NHS and private operating lists. He deals with injuries to the hand in his trauma clinic at the Royal Victoria Infirmary and he teaches and trains regularly on plastic surgery and hand surgery topics including on internationally recognised courses.


Mr Williams has over 25 years of plastic surgical experience. He was appointed as an NHS consultant at the Royal Victoria Infirmary in Newcastle upon Tyne in 2000 after completing specialist training in both plastic surgery and hand surgery. He has been clinical audit and governance lead for a 10 years period which has progressed into my being appointed head of department in 2016 which he continues to date.


Full member of the British Association of Plastic, Reconstructive and Aesthetic Surgeons (BAPRAS)  and the British Society for Surgery of the Hand (BSSH). On the specialist register for plastic surgery and has full training in cosmetic surgery.

hand surgery

I undertake all aspects of hand surgery with the exception of wrist injuries. My main clinical interests within hand surgery relate to Dupuytren’s disease, small joint replacement, tendon and peripheral nerve injury including tendon transfer.
My PhD was by research into the metacarpophalangeal joint of the hand and its replacement.

 I have produced over 1500 Reports.  I undertake both Personal Injury and Medical Negligence work.  I have attended training courses on medico-legal report writing. I am instructed to prepare reports both for Defendant and Claimant.

Considering Cosmetic Surgery

 

The decision to have cosmetic surgery should not be made lightly. You need to be honest with yourself about what you want to have done and what you expect to achieve. The decision about surgery is yours to make and you should spend your time researching and looking for information specific to the surgery you are considering. You need to remember however that often web sites are there for advertising purposes and you need to remember that no surgical procedure is 100% risk-free. You should always seek advice from an experienced trained surgeon who will be undertaking the surgery and feel comfortable with the surgeon you have chosen. There are two organisations associated with the Royal College of Surgeons which have strict guidelines about membership. These are the British Association of Plastic, Reconstructive and Aesthetic Surgeons (BAPRAS) and British Association of Aesthetic Plastic Surgeons (BAAPS). Membership of one or both of these professional bodies will indicate that the surgeon has had advanced training and attained appropriate recognition. You may also get further assurance in finding that your surgeon is also a consultant in the National Health Service, as to be appointed to such a post does involve significant training and experience. 

The surgeon will assist you to help you decide whether the surgery which might be offered will address your needs appropriately and you will need to balance the benefits against the risks. You should not let anyone talk you into having something done for which you had little in the way of concern before the consultation.

You should not travel a long distance to have surgery and you need to be sure of what arrangements are made for follow up and the management of any problems or complications if they were to arise. 

Cosmetic surgery needs careful thought. The procedures are can be very complex and you must be certain after discussing the proposed procedure with your surgeon that the issues you have will be helped by the procedure. Whilst overall cosmetic surgery is safe there are many things you need to consider such as whether the resultant scars will be acceptable to you and whether the minor drawbacks which can happen with any surgery, are worth the risk to achieve what you would like.

Breast

Breast AUGMENTATION

Breast augmentation is a procedure used to increase or restore the size and shape of a woman’s breasts.

A breast augmentation improves your physical appearance and also your sense of femininity. The implants are placed either beneath the breast in front of the muscle or behind the muscle itself. Which method depends upon your physical characteristics. The exact size of the implant and its projection depends on many  factors not only your preference but also the physical appearance of your breasts. 

The final decision on implant size and shape is made in consultation with the patient.

The operation is undertaken under a general anaesthetic with the incision being close to the fold  underneath the breast. The incision is approximately 5 cm in length. A pocket is developed beneath the breast or the muscle and the implant placed in the space created. The incision is closed with a single continuous skin stitch, covered with a small dressing and a comfortable sports bra applied. The stitch is simply trimmed 14 days later.

BREAST REDUCTION

A breast reduction is an operation  reduces the size of the breasts. The size of women’s breasts may be determined by several factors, including genetic reasons, body weight and hormonal influences.  Large breasts may cause backache, neck pain, grooves in the shoulders from bra straps, rashes under the breasts and the feeling of self-consciousness. In addition to reducing the size of the breast, the shape is also improved with the breast being lifted. The choice of reduction technique depends on the physical appearance of your breasts.

The operation is undertaken under a general anaesthetic and often requires a 2 night stay in hospital.

In order to re-shape and to give a lift to the breast this inevitably gives rise to scars. 

The stitches are dissolvable but will need trimming at 2 weeks.

A well fitting sports bra will be required to be worn post surgery

BREAST LIFT

A breast lift (mastopexy) is designed to correct the shape of a breast that has developed droopiness (ptosis) and restore the breast and nipple to a more “normal” or youthful anatomical position. The technique used depends on several factors, including the quality of the skin, the degree of volume lost from the breast and the amount of excess skin. Only a small amount or no breast tissue is removed at the time of surgery to give the best shape. The tightened skin lifts up and supports the breast tissue, but in order to tighten the skin and to give a lift to the breast there is a need to remove some skin and this inevitably gives rise to scars. In order to lessen their impact they are usually placed around the nipple and underneath the breast.

BREAST LIFT WITH AUGMENTATION

An implant is can be used in conjunction with a breast lift and can usually be undertaken at the same time. The implant helps to provide projection and fullness of the breast when fullness in the upper part of the breast and in the cleavage area is required. Sometimes ladies requesting breast implants may be advised that a combination of breast lift and an implant would be more likely to give the result they desire. Patients who have loose skin or whose nipples have dropped significantly following pregnancy will usually be the ones to benefit from this type of procedure.

The effects of aging on the face produces the characteristic appearance of the jowls, wrinkling and loss of fat which leads to loose skin and hollowing. The aim of a facelift is to rejuvenate the face and give a more youthful appearance. There a various facelift techniques and which one utilised depends upon the patient desires. 

The use of additional procedures such as fat transfer and liposuction can be used to achieve a better contour.

  Wrinkled, hooded skin, often with prominent eye bags may occur with facial aging or in some patients who have a family history of the problem. Eyelid (blepharoplasty) surgery is performed on the eyelids and is designed to produce a youthful and more rejuvenated appearance. The surgical procedures differ for each of the eyelids and the procedure is tailored to each patient.

Abdominal Surgery

An abdominoplasty or "tummy tuck", is an operation to remove excess skin and fat from the abdominal wall. The type of procedure required depends on the individuals existing characteristics. Frequently the abdominal wall muscles are weakened producing an abdominal bulge and often a muscle tightening procedure is also undertaken. An abdominoplasty improves the waistline, and produces a flattened tummy. The resultant scar is placed low on the abdominal wall which helps it being concealed beneath underwear.

Liposuction is a procedure to remove unwanted fat deposits. It is not a procedure to lose weight. Excessive fat deposition often occurs around the hips and abdomen, inner and outer thighs and the knees. 

The procedure is usually undertaken under general anaesthetic and a cannula connected to a vacuum is used to remove the fat. The skin will usually shrink back but in some situations, where there is skin excess, this may not occur. In this situation surgical excision will be recommended along with the liposuction. There is often bruising following the surgery which can be uncomfortable. The bruising usually takes a few  weeks to clear. Pressure garments are required to be worn to help mold the treated area to gain the best appearance for usually a 6-8 week period.

Tendons can be injured requiring repair or splintage.

Degenerative change can also occur producing locking of fingers and discomfort which can be treated by injections or surgery

Dupuytren’s disease is a common condition where abnormal scarring appears in the palm of the hand which can prevent the finger from fully straightening.

 There is as yet no cure. Surgery is usually advised to make the fingers straighter, but it may not always lead to full straightening of the finger and it will not eradicate the disease.

Arthritis can affect any of the joints of the wrist and fingers, particularly the base of the thumb and the end joint of the finger. Some degenerated joints can be replaced or permanent stiffened to eradicate pain and improve hand function.

A degenerated joint can also form ganglion cysts which if problematical can be surgically removed or aspirated.
